FAMILY STABILIZATION, WRAP AROUND, FAMILY PRESERVATION & TARGETED CASE MANAGEMENT

 

These family based programs are based in Burlington United Methodist Family Services, Inc.’s office in Oakland, Maryland.  The programs are made possible through a grant secured through the Garrett County Partnership for Children and Families, Inc.

FAMILY STABILIZATION:

The Family Stabilization program provides in-home services to families who are identified as needing services to increase individual and family functioning and reduce the risk for more intensive services.  The program involves a collaborative effort among educators, social service agencies, health care providers and the public school system.

WRAP AROUND:

The Wrap Around Case Management Program is a voluntary, long-term, in-home intervention that targets children, youth and families who have been repeatedly involved with various community resources and have yet to maintain self-sufficiency.  The focus of the program is to help improve family functioning and self-sufficiency.  Services include comprehensive case management, teaching of basic living skills by a case aide, 24-hour crisis intervention, supportive individual and family counseling, parent and family education.

FAMILY PRESERVATION:

The Family Preservation Program is voluntary, intensive, in-home intervention for families who are at imminent risk of having children placed out of the home.  This strengths-based program works with the entire family to improve family functioning and to strengthen parents’ capacities to care for and protect their children.

TARGETED CASE MANAGEMENT:

This service is intended to assist a client with identifying and obtaining needed services such as mental health treatment, medication, Federal and State benefits, housing, etc.  Adult services are provided to adults 18 years and older who meet one or more of the eligible criteria:  serious or persistent mental disorder; poor functioning in the family and community; repeat hospitalizations; multiple physical and/or mental disabilities; homeless or in danger of losing current residence; and recent incarcerations or on parole or probation.

Access to the above programs is through referral by the Maryland Department of Human Services in Garrett County.
